If you would like to make an appointment, please contact our office by phone at (615) 451-4306. We can usually accomodate same day appointments for new patients. Walk-in appointments are welcome for existing patients.

If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice. We ask for this advance notice so that we can offer this appointment to another patient.

We will be more than happy to submit all insurance forms for you and help you recover the most from your benefits. We will do everything we can to help you afford the treatment you need and want. Forms of payment accepted by the office are check, cash, or credit card (visa, mastercard, discover). Payment is due at the time of service unless prior arrangements have been made.

We accept most traditional insurance plans including but not limited to the companies listed below. We also accept Personal lnjury and Worker's Compensation cases. Please contact our office to verify acceptance of your plan. Qualifications for insurance coverage may differ due to the uniqueness of each procedure.
